Nerve Monitoring During Brain Surgery

چکیده

An area of special challenge, is the cerebellopontine angle (CPA), which is formed between the brain stem and the cerebellum, because of the close proximity of major nerve trunks and blood vessels. Acoustic tumours account for approximately 80% of CPA tumours, and surgery involves risk primarily to the facial nerve the motor nerve of facial expression, and the cochlear nerve the sensory nerve of hearing. These tumours, which arise from the sheath of the vestibular nerve the balance nerve, produce symptoms including gradual deafness, tinnitus, and balance disturbance, on the affected side.

Facial Nerve Monitoring During Parotidectomy: A Two-Center Retrospective Study

Introduction: We present a retrospective two-center study series and discussion of the current literature to assess the benefits of facial nerve monitoring during parotidectomy. Materials and Methods: From 2007 to 2012, 128 parotidectomies were performed in 125 patients. Of these, 47 procedures were performed without facial nerve monitoring (group 1) and 81 with facial nerve monitoring (group 2...

Ancient Schwannoma of the Trigeminal Nerve Mimicking High Grade Lesion

Ancient schwannomas rarely found in the central nervous system and ancient change in an intracranial schwannoma (intracranial portion of the trigeminal nerve) is extremely rare. A seventy-year man presented with headache of 3 months duration and one episode of generalized tonic-clonic seizures. Imaging findings were suggestive of left temporal high grade lesion. During surgery there was well...

Electrophysiological monitoring during acoustic neuroma and other posterior fossa surgery.

Techniques used to monitor the function of the seventh and eighth cranial nerves during acoustic neuroma and other posterior fossa surgery are reviewed. The auditory brainstem response (ABR), electrocochleogram (ECochG) and direct recording from the auditory nerve (CNAP) were compared. The best technique is the ECochG, although in many cases, the CNAP should be used as a back-up technique. The ...

Detection of evolving injury to the brachial plexus during transaxillary robotic thyroidectomy.

OBJECTIVES/HYPOTHESIS Continuous intraoperative neuromonitoring (IONM) of transcranial electric motor evoked potentials (tceMEPs) and somatosensory evoked potentials (SSEPs) has gained universal acceptance as an efficacious method for detecting emerging positional brachial plexopathy or peripheral nerve compression during spinal and shoulder surgery. This has implications for transaxillary thyr...

Role of Intraoperative Nerve Monitoring During Parathyroidectomy to Prevent Recurrent Laryngeal Nerve Injury

Injury to the recurrent laryngeal nerve (RLN) is a well known, though less frequent, complication of parathyroid surgery. In recent years, the use of intraoperative nerve monitoring (IONM) has gained popularity amongst surgeons when operating on thyroid gland; however, its utilization in parathyroid surgery is not established. This trend continues to rise, despite multiple studies documenting n...
